What happens during your I-693 exam

USCIS and the CDC set the exact scope of the immigration medical examination. Here is what a complete exam covers, and what we perform at NY MedCare.

Physical Examination

A head-to-toe examination including eyes, ears, nose and throat, heart, lungs, abdomen, lymph nodes, skin and extremities, plus height, weight, blood pressure and vision.

Tuberculosis Screening (IGRA)

We order an Interferon Gamma Release Assay blood test, required by the CDC for applicants aged 2 and older. The older PPD skin test is no longer accepted as the initial screen.

Required Blood & Urine Labs

Syphilis serology and gonorrhea testing within the CDC-specified age ranges. Ordered at your visit and processed by the laboratory we direct you to.

Vaccination Record Review

We review your immunization history against the CDC schedule, identify exactly what is missing for your age, and write the prescriptions so you can complete them at your pharmacy.

Mental Health & Substance Screening

A confidential evaluation for mental-health conditions associated with harmful behavior and for drug or alcohol use disorders, as USCIS requires.

Completed & Sealed Form I-693

Vaccine Requirements

Which vaccines USCIS may require

Which ones apply depends on your age and immunization history. We identify exactly what you need and write the prescription.

MMR (Measles, Mumps, Rubella)

Required for most adults and children. We verify documented dates or order laboratory blood titers to prove existing immunity.

Two recent changes worth knowing. Polio (IPV) was added to the CDC technical instructions for civil surgeons in May 2024. The COVID-19 vaccination requirement was removed by USCIS effective January 22, 2025. You no longer need to document it on Form I-693.

Lost your vaccination records? You still have options.

This is one of the most common situations we see. Depending on your age and history we either prescribe the vaccines you need or order blood titers to prove you are already immune. Either route completes the vaccination section of your form.

What exactly is Form I-693?

Form I-693, Report of Immigration Medical Examination and Vaccination Record, is the official USCIS form used to document that an applicant for immigration benefits is not inadmissible to the United States on health-related grounds. It must be completed and signed by a USCIS-designated civil surgeon.

Is Dr. Irshad a USCIS-designated civil surgeon?

Yes. Dr. Huma Irshad, MD is officially designated by U.S. Citizenship and Immigration Services (USCIS) as a civil surgeon authorized to perform immigration examinations and sign Form I-693.

How long does the appointment take?

Your exam appointment typically takes about an hour. During this visit, Dr. Irshad performs your physical exam, reviews your vaccination history, and writes all required lab orders and prescriptions.

Do I need a TB skin test or a blood test?

Per CDC technical instructions, all applicants aged 2 and older must receive an Interferon Gamma Release Assay (IGRA) blood test (such as QuantiFERON-TB Gold or T-SPOT). The older PPD skin test is no longer accepted as the initial screening.

Which vaccinations does USCIS require?

Required vaccines depend on your age and medical history, including MMR, Tdap/Td, Polio (IPV), Varicella, Hepatitis A & B, Rotavirus, Hib, Meningococcal, Pneumococcal, and seasonal Influenza. Note: The COVID-19 vaccine requirement was officially removed by USCIS effective January 22, 2025.

What if I have lost my vaccination records?

This is very common. Dr. Irshad will evaluate your needs and can order blood antibody titers to prove immunity for diseases like Measles, Mumps, Rubella, Varicella, and Hepatitis B, or administer needed immunizations.

Does my completed Form I-693 expire?

Under USCIS policy updated in 2024, a Form I-693 properly completed and signed by a civil surgeon on or after November 1, 2023, retains evidentiary value indefinitely and does not expire, as long as it was filed concurrently with your immigration petition.

Which edition of the form should I bring?

You must bring the current 01/20/2025 edition of Form I-693. USCIS has accepted only this edition since July 3, 2025. Please fill out Part 1 before your visit, but DO NOT sign it until instructed by the civil surgeon.

Can I open the sealed envelope?

Never open the sealed envelope. USCIS will reject your Form I-693 if the seal is broken or tampered with. We provide an identical unsealed copy for your personal records.

Does insurance cover the immigration physical?

Immigration medical examinations are generally not covered by health insurance and are self-pay. However, insurance or prescription coverage may often be used for third-party lab work or pharmacy vaccines.

Do you serve applicants from outside Manhattan?

Yes. We serve applicants from all five NYC boroughs (Manhattan, Brooklyn, Queens, Bronx, Staten Island), Long Island, Westchester, and across New Jersey and Connecticut.

Do I need to be an existing patient or have a referral?

No. You do not need to be an established patient of our clinic, and you do not need a referral from any attorney or physician to schedule an exam.
